GBR PROCEDURE OF A BUCCAL-PALATAL COMMUNICATING DEFECT ON MAXILLA PLUS SINUS LIFT.

I present a case of a patient without relevant medical records, only smoking habit, but we achieve him quitting 4 months before the surgery and never starts again after that, with a buccal-palatal defect at his second quadrant, and a complete loss of integrity in the anterior wall of the maxillar sinus that made impossible any implant placement and posterior rehabilitation with fixed prosthesis.

It´s not a common case, and there are different approaches. I choose to perform a sinus lift combined with GBR to resolve the communicating defect and provide enough width to the future implant placement.

I use a combination of several collagen resorbable membranes with a mix of 60% of autologous bone and 40% of xenograft.

I also use several optional elements, not critical to the predictability of the treatment, but very helpful to achieve stability of the complex, such as a titanium mesh at the sinus wall osteotomy and a xenogenic cortical lamina to restore the palatal wall.

After ten months, we perform a reentry surgery. We achieve a hard ,vascularized bone and proceed to place 6 implants for the future fixed prosthesis.

We will shortly publish the follow up, with the soft tissue management, prior to the prosthesis confection.
