Difficult case of esthetic on implant in anterior area

Patient came with an old implant and he wanted improved esthetic.The case was approached in 6 stages:
1. Decoronation- removal of crown and abutment in order to allow a growth of soft tissue
2. Removal of old implant and soft tissue graft(onlay graft)
3. Bone graft
4. Implant placement using a guide
5. Temporary crown
6. Rotated palatal flap to reconstruct the papilla between central and lateral incisor
7. Or tho to create the occlusal space for final crown

The most important factors that influence the esthetic outcome of the implant-supported restorations are as follows:

- Patient selection and smile line;
- Tooth position;
- Root position of the adjacent teeth;
- Biotype of the periodontics and tooth shape;
- The bony anatomy of the implant site;
- The position of the implant.
