Complex case with big augmentation using only autogenous bone

Patient, 50 years old, wants to have fixed prosthesis supported by implants. She is a heavy smoker so a lot of surgeons didn’t want to put her implants and bone graft.
I had a 3 stage approach:
1. Implants on the upper for a temporary fixed prosthesis in order to proceed with the bone augmentation in the frontal area.I had to remove bone and soft tissue on the right side for symmetry and esthetics.
2. Autogenous bone grafting in anterior area using bone from the external oblique line in mandible( Khoury method)
3. Implants in the frontal area and then fixed prosthesis.

Initial/temporary/final
Initial with no prothesis( note the assimetry)

Bone augmentation
X-ray with temporary in place
