3 rooted premolar?

I'm looking for advice if this might be a 3-rooted second premolar? Normally I'd do a CT but the nearest one is over 50km away and the patient can't drive and there are no trains or other forms of public transport.

If you think it is 3 rooted and the 3rd root is as bent as I imagine, is the 3rd one usually located buccally or palatally?

Otherwise where would the periapical change be originating from?

Many thanks. A lot depends on that tooth.
