What is the best solution ?

25 year old patient derived from colleague to make connective tissue graft. Implant placed 3 years ago with progresive gingival recession.

thinking of:
1.removing actual crown and abutment
2.maryland provisional
3.rotated palatal connective tissue graft and guided bone regeneration with khoury or titanium mesh to
reconstruct buccal bone
4.new abutment and provisional for soft tissue re contouring (perhaps new connective tissue graft)
5- final prosthesis after evaluating tissue response.

thoughts ?
what about implant position ?
Considerations about removing it ?

Difficult case but you have a good plan. Always best to remove corwn and go to cover screw and allow 2-3 weeks for tissue growth over top...then CTG...then decide. Dr. S
