one year ago.patient went to dentist with swelling in right area. unfortunately dentist gave him antibiotics"injection" and tablets but it does not work although patient finish AB course.few days later and swelling release itself.few months later swelling rebirth again.patient went to dentist and dentist decide to extract upper right 5 but it does not work too as theses days swelling appear again and patient came with pain with pain with hot and cold water and during eating . panorama show this details. what is your opinion

If there is no dental pathology, and swelling is on the inner part of cheek, could be Stenon´s conduct obstruction.

After checking parotid and thyroid lobes it show normal results

I´m confused, you said right but show more the left side on rx pan....is it right?
Xrays are not clear enough for diagnosis.

Thank you for your notes .I just captured panorama from back side but swelling is on site of extraction of upper 5

did you take a bitewing radiograph and do percussion test?
maybe it is from the lower teeth.
