Dear Dr. Zhang,
There is no clarity on what is considered a large incisive canal and when it should be called a Nasopalatine duct cyst radiographically. If this patient is asymptomatic, has no fluctuant palatal swelling, and the lesion/structure is not going to interfere with any procedure planned in the area, the approach should be one of periodic radiographic recall and surgical intervention if there is signs of expansion. Typically, these cysts have a slow expansion rate unless infected .
