May I place implant without extracting the neighbour tooth

Dear my colleagues,
My patient lost tooth 47 (right second molar) because of periodontal disease. This tooth was taken out 01 year ago. Now he wants to place 01 implant for this area. The problem is that the neighbor tooth has no bone at distal root (on the Xray)but patient doesn't want to take out this tooth. Now he feels no pain, good function and he scares about extraction.
So can I keep this tooth and just place 01 implant at tooth 47? If not, how can I convince patient about extraction.

HI. if your pa radiography was parallel decision for treatment plan was better. but in this view it seems because of advanced bone loss on distal root, short roots and endodontic problem in mesial root, indivisual prognosis of second molar is poor that means should be extract during implant surgery. you have 2 case presentation that case presentation to patient that who want to save teeth is very difficult.in otherwise you must think about third molar that has a mesial tilting.

Thank you very much for your in consultation. Certainly, I'll ask patient to take a conebeam CT to confirm the bone quantity and quality. However, in my opinion, it's difficult to keep the tooth 46. Do you think the root planning will be effective for this case?

without a c.t scan I wouldn't think about anything.
How can I gather information with a bidimensional Xray and probably not centric placed.
I would recomend to perform a C.T scan and then deside if you can do one implant ,if the neighbour tooth has to be extracted??????

It's not a good treatment plan to insert the dental implant near a tooth with that amount of bone loss
The medial roots of this tooth is also having a lesion I think.

That's right,the patient should be informed about the circumstances totally,about the benefits ,and about longterm results of installing a dental implant near that tooth,tooth number 46 might be maintainable but the question is that if it's beneficial for the patient to save it!
