Hello friends,
Patient presents with a significant complication in esthetic zone. Implant #7 was placed about one year ago. patient reports losing papillae between 7,8 afterwards. Smile line is relatively high and shows everything. Patient is not interested in orthodontic extrusion. He knows this will never look perfect and has realistic expectations of the final result.
How would you manage??

Hello Ehab, seems to be carious lesions around crowns on #6 and #8. Therefore maybe remove implant, temporise with bridge from 6 to 8, soft tissue graft and permanent bridge, or move on to bone grafting and new implant depending on what patients needs are...? Interesting for sure.

No chance of a decent esthetic result without ortho extrusion on central incisor. Try to encourage him to reconsider.
I would take out implant here and graft with bone and tuberosity tissue....then either bridge or new implant. regards Dr. Salama
