55 year old female with non contrib med history referred by oral surgeon. He extracted 30 and grafted . Graft failed with puss and infection. He then removed graft and regrafted with prp.
He then implanted . Implant failed . This is over 2 years .


Patient has daily localized acute pain in 30 position .

Clinically tissues are healed . Cone beam reveals small fragments of
Too tip .

Adjacent teeth are vital .

Check for Bisphosphonate usage over past 5 years? Poor healing here seems may be related to medical condition, diabetes, medication induced, osteoporosis Tx, autoimmune.....and of course TRAUMATIC extraction with lack of complete removal of tooth fragments and infected tissues. Would recommend through medical workup and blood tests. Only then, re-open and remove all remnants WITHOUT any additional grafting....only PRF/PRGF. regards buddy, Mo
