55 years old patient , wanting implants placement in posterior right and left mandibles.

She was treated some time ago with a xenograft , but didn´t work.

We planned 2 3D bone grafts following Pr Khoury´s approach.

We had to be careful as because you can see, the lamina oblicua on both sides weren´t too thick , so apical cut had to be under both IANs , as we knew were going to be exposeds at the moment of takingthe graft out.

Surgery was done under IV sedation and with DENTSPLY Micro Saw kit , the graft was fixed with Meisinger 1mm microscrews.

Superb William, Please keep us posted with the results. Do you fill the donor sites after taking the plates? You should try Stoma microscrews, they have much neater heads. What is your plan for soft tissue?? Regards

Thanks very much Andoni I just placed some collagen sponges, I usually don´t fill the donor site´s space , much less in this case because NDI was fully exposed, so biomaterial can create hyperestesia with big pain associated. About soft tissue, the idea is to do Kazanjian technique to create fixed gingiva. Thanks once again, best

Dear Dr Salama, Thanks very much, Very important As words coming from you, as you know, very good passivation is needed. It's true anyway that if you've got very thin mucosa, heads of screws can open the wood.Hope not in this case. Of course I'll poste the result in 4 months time Thanks once again, best
